You must return it to us on or before the first day of any free or paid program with Jump Athletic. You can do one of three things:
- Fax it to 602-357-3405
- Scan and email to info@jumpathletic.com
- Bring it the first day of training academy or practice
Reminder: We must have a signed copy in hand the first day of your child’s training with Jump Athletic. Call us at 602-540-7669 if you have any questions.
